Thank you for your interest in submitting a review of 2 Ears 2 Learn. Reviews of this nature are a little more complex than the standard Google Review as we are dealing with information protected by HIPAA (Health Insurance Portability and Accountability Act), FERPA (Family Educational Rights and Privacy Act), COPPA (Children’s Online Privacy Protection Act), and possibly other laws. With this in mind, we require all submissions of reviews be accompanied a signed copy of our standard “Photo, Audio, and Video Release Form (for Minors)” to protect all parties involved.

As a standard of practice, and to comply with numerous laws and regulations, we will only identify a child by their first name and last initial (For example: Sarah E.) If any photo, audio, or video is submitted with your review, it will only be displayed if the required documentation is submitted (see below). Photo, Audio and Video Release Form (for Minors)

I, the undersigned, hereby grant permission to 2 Ears 2 Learn, its officers, employees, agents, and assigns (“2E2L”), to take, use, reuse, publish, republish the below named minor child’s likeness (photographic, video-form or otherwise) and voice, for the purposes of advertisement, fundraising, publicity, promotion, trade or any other use by the 2E2L, in any and all forms, media and manners, including but not limited to, news releases, websites, social media, photographs, video, marketing and exhibition for an indefinite period of time.

I warrant that I am the parent or legal guardian of the below named minor child and have full right and authority to enter into this Release on behalf of the undersigned.

I understand that these images and voice recordings may be used for a variety of purposes, as outlined above, without further notification. And as such, I further understand and agree that neither I, nor the undersigned minor, have any right to inspect, or approve of the finished photographic, video, or audio recorded products.

I acknowledge that no compensation, fee or royalty will be paid for above mentioned uses of the minor child’s likeness or voice and that the 2E2L owns all rights to the images, videos and recordings, and all derivative works created therefrom. Nor will I seek compensation, on behalf of myself, the undersigned, or any other party.

I release 2E2L, its, officers, employees, agents, and assigns from any and all claims, demands, damages and liabilities arising out of, or in connection with, the use or distribution of said photographs, video or audio recordings, including but not limited to, claims of invasion of privacy, defamation, or infringement of copyright, moral rights, or rights of publicity.

I understand that this consent is perpetual, that it may not be revoked, and that it is binding on me, the undersigned minor, our heirs, assigns or any other third party.

This Release expresses the complete agreement of the parties and supersedes all prior communications, contracts, or agreements between the parties with respect to the subject matter of this Release, whether oral or written.
